How to Disclose
Initial disclosures are made through Cayuse as part of the pre-award process via the Cayuse Outside Interest system.
Questions on the form will lead you through disclosure requirements. Disclosures are required at the initial submission of a grant proposal, annually thereafter for funded projects, and on an ad hoc basis within 30 days of acquiring a new interest. Disclosures are reviewed to identify potential conflicts of interest. Any conflicts of interest that are identified must be managed or eliminated within 60 days.
Steps for Completing a Disclosure
- To access the Cayuse Outside Interest system, please visit: https://illinoisstate.app.cayuse.com/.
- From the home screen, select the drop down arrow in the upper right-hand corner of the screen next to your name. Then select “My Profile”.
- On the left side of the screen, select "COI Disclosures" from the dropdown menu.
- In the upper right corner of the screen, select the "New Disclosure" button.
- You will be prompted to select either a “Research-Based disclosure” or an “Annual disclosure.” Select the appropriate disclosure and then select the next button.
- Answer all required questions that appear in the form.
- Once you have answered all required questions, a green checkmark will appear next to all of the sections on the left.
- In the upper right corner of the screen select the blue Submit button.
